Health Forms
Before you come to campus, the University requires that you complete a total of three health forms: The Immunization Form, the Meningitis Response Form, and the General Health Form. According to the Office of Student Affairs, the deadlines for completing these forms is as follows
- The Immunization Form must be submitted three weeks prior to your Orientation/Enrollment date.
- The Meningitis Form must be submitted at least three weeks prior to your Orientation/Enrollment date.
- If you are 18 years of age or older, this requirement can be met by logging on to your SOLAR account and submitting your response electronically under "Holds."
- The Health Form must be submitted before the first day of classes.
Bear in mind that the first day of the classes for the 2019-2020 Academic Year is Monday, 26 August, 2019
