Before you come to campus, the University requires that you complete the Stony Brook Immunization Form. In the form, you will be required to show:

  • Proof of immunity to measles, mumps, and rubella (MMR). You can find more information about the MMR requirement here.
  • Proof of vaccination to meningitis OR acknowledgement receipt of information on meningitis. You can find more information about the meningitis requirement here.

The university strongly encourages the vaccination against COVID-19 (and the subsequent boosters) if possible.

You will need your primary care physician to complete all the forms. Within 10 days of receiving your NetID, you should submit the form to the Wolfie Health portal to remove the enrollment hold on your account.

If you are an international student and your immunization records are in a language other than English, you can still submit your documentation, but allow the Student Health Center 10 days for translation. You will not be charged for translation.

Once your documentation is processed by the compliance team, the status of your submission will be shown as "verified" under the Wolfie Health Portal's Immunizations tab.
