Health Forms

Before you come to campus, the University requires that you complete a total of three health forms: The Immunization Form, the Meningitis Response Form, and the General Health Form. According to the Office of Student Affairs, the deadlines for completing these forms is as follows

• The Immunization Form must be submitted three weeks prior to your Orientation/Enrollment date.
• The Meningitis Form must be submitted at least three weeks prior to your Orientation/Enrollment date.
  • If you are 18 years of age or older, this requirement can be met by logging on to your SOLAR account and submitting your response electronically under "Holds."
• The Health Form must be submitted before the first day of classes.

Bear in mind that the first day of the classes for the 2019-2020 Academic Year is Monday, 26 August, 2019

Please see your primary care physician (regular doctor) at least three weeks before this date so that you may complete all your forms on time. If you turn your forms in late, it's not the end of the world, but you will not be able to register for classes for the Spring semester until you submit your health forms.
